The Rules Your Pet abides By:

	I am familiar with my local animal control laws.
	All family members have agreed to this adoption.
	I understand that if at any time during the life of this pet, if I am unable to keep it, that I must return it to Furever After Rescue at my own expense? I will not take it to an SPCA, Animal Services, Rescue Shelter, give it away to another individual or group, or sell it to another individual or group.
	I understand that I will be expected to provide for this animal with needed medication and/or preventative medications.
	I understand that I will ALWAYS have the opportunity to conact Furever After Rescue after the adoption to ask question, obtain training assistance or guidance.
	I understand that the adoption donation is $350-450, to cover costs and/or any extensive medical treatment, (unless specified otherwise).

Terms of Agreement:
I have read the above information carefully and have filled out this application honestly. I understand that omission of information and/or failure to answer all questions can result in this application being declined. Also, if an omission or untruth is discovered after an adoption takes place, I understand that Furever After Rescue Society reserves the right to annul the adoption and reclaim the animal. I give Furever After Rescue permission to fully investigate the information provided as well as contact veterinarians, and related officials. I agree to a home and a yard visit on a mutually agreed date by Furever After Rescue or a person on their behalf before an adoption decision is made. I also understand and agree that Furever After Rescue shall have the right to conduct a follow-up visit, on a mutually agreeable date after the date of adoption. Furever After Rescue may attend in person or shall have a person attend on their behalf. In addition, I understand the adoption decision is dependent on many factors including but not limited to the compatibility of the family and home to the individual animal, and other applications received on this animal. I understand it is the Furever After Rescue’s prerogative to decide which home is most appropriate and that their decision is final. Unless otherwise indicated by Furever After Rescue, I may be considered for another more appropriate animal in the future .
